POLICIES

 

NEW PATIENT APPOINTMENTS

At this time, PSOT accepts doctor referrals only. This means that your primary care physician or other specialist will need to make a formal referral to one of our physicians.

If you have a respiratory problem or sleeping disorder you will need to see your local physician. At that time, your physician will make the decision to refer you to a pulmonary physician. Their office will call us to set up the appointment.

FOLLOW-UP APPOINTMENTS

Follow-up appointments are made as needed. Any problems other than those that are directly related to pulmonary medicine will need to be addressed with your primary care physician. PRESCRIPTIONS & REFILLS

Medications are prescribed as deemed necessary by the physician. It is our policy that we do not prescribe medications without assessing the patient in the office. This will require a call to our office to set up the appointment.

All refills are handled through our prescription call-in line Monday through Friday, 8:00 AM to 5:00 PM. Just have your local pharmacist call our pharmacy line and give your name, date of birth, the name of your medication, strength and last date filled. This will go into a pharmacy mailbox. These calls are downloaded frequently during the day. After confirming the information in your chart (or with the physician), the pharmacy will be notified. Please call your pharmacist before picking up your medication to make sure it is ready.

FEES

We ask that you pay for office visits at time the service is rendered. If you have insurance that our physicians participate in, we will accept your co-pay and/or deductible. If there are extenuating circumstances, our clinic administrator will need to be notified.

INSURANCE

We do participate in most of the HMO's and PPO's in the area but just remember, THE PATIENT IS RESPONSIBLE FOR KNOWING HIS/HER OWN INSURANCE PROVISIONS. PLEASE CHECK WITH YOUR INSURANCE CARRIER BEFORE COMING TO OUR OFFICE TO MAKE CERTAIN WE PARTICIPATE IN YOUR PARTICULAR PLAN. Also, it is your responsibility to obtain pre-authorizations or pre-certifications that may be needed for you to be seen in our office. If you have questions, please call our office.

Medicare patients will be expected to pay the deductible and the 20% of charges made in the office. For those who have a Medicare supplement, this can be filed through our office.
