Chrome is known to have issues filling out online PDF forms. It is highly recomended that you use a different browser, such as:
- Internet Explorer
- Mozilla Firefox
- Apple Safari

Alternatively, you could try to configure Chrome yourself. (ADVANCED USERS ONLY)
- Type "chrome://plugins" in the address bar (without the quotes)
- Hit enter
- Click on details (near the top right)
- Disable "Chrome PDF Viewer" and enable the Adobe one
